Preface
I am Dr. Md, Shamsuddin Civil Surgeon, Sirajgonj . Sirajgonj is the Gateway entry of North Bengal which is situated at the bank of the river Jamuna. Sirajgonj District has 9 Upazila Health Complexes. It has 82 Unions and 6 Pourosavas. Its Population is 30,82,256. Its area is about 25000.44 square kilometer.. I am highly pleased and proud that Health Bulletin containing Health related data and informations are going to be published through out the country. Undoubtedly it is a great initiative of DGHS particularly of MIS section of DGHS. I hope that it can satisfy the demand of Health related data and informations seeking by the people. I am also very glad to publish Health Bulletin of Sirajgonj District containing Health related information of 2014. I think, the data of Health Bulletin collected from different Health Centers and field staffs which will fulfill the need of publication of Health Bulletin. Finally, I would like to congratulate the initiator of the Health Bulletin software for setting us one step ahead towards Digital Bangladesh.

Important information and necessary issues for Serajganj Civil Surgeon Office
Sirajganj is one of the important Districts of the country and it comprises of 09 Upazilas. Here it is needed to create 02 ( two) posts of Additional Civil Surgeon class one statistician post . • Routine EPI and EPI diseases surveillance are going with good shape and District is free from wild poliomyelitis for more than last 14 years in comparison of the Country for the last 7 years. Right now . • One Vehicle for Deputy Civil Surgeon is needed that will be helpful for monitoring of the District Program activities. • EPI equipments cannot be stored in present store room and Office Staff/ WHO/ Project Staff cannot be accommodated properly due to shortage of rooms . To solve this problem half part of 2nd floor should be extended immediately vertically. • For maintaining routine and daily work all Upazila MT EPI and District EPI Superintendent need one computer each and all Upazila also need at least one computer and one printer each. • Civil Surgeon Office needs more 3 (three) Des Top Computer and 1 (one) Photo Copy Machine to maintain compose job smoothly. • If those facilities will be delivered to this office, the administration & general work will run properly.
