Preface

It is a great pleasure for me to bring out the Health Bulletin 2016 of NIDCH. This is the only tertiary level center in the country for management of complicated TB and Chest diseases of Bangladesh. It was established as TB hospital in 1955, upgraded in 1962 as Institute of Chest Diseases and Hospital and started Post-graduate course Diploma in Tuberculosis Chest Diseases and it was the first Post-graduate Institute in Dhaka. It was upgraded in 2002 as National Institute of Diseases of the Chest and Hospital. Introduction of MD course in Chest Diseases in 1992 and MS in Thoracic Surgery in 1999 are the mile stone for this institute in the field of respiratory medicine & thoracic surgery human resource development. Subsequently FCPS in Pulmonology and Thoracic Surgery courses introduced & these are also a further addition for development of human resources of the country. Establishment of National Asthma Centre bring the asthma and COPD management under a uniform National Guideline. Establishment of National Tuberculosis Reference Laboratory strengthen the Tuberculosis Control Programme through appropriate diagnosis and management of MDR-TB patient. Introduction of CT Scan further improved the diagnosis of the patients and thus made patient management earlier and more perfect This institute is also a main center for implementation pilot project DOTS-Plus for management of MDR-TB Patient. Poly-somnography, DLCO and NO measurement facilities,Body box EBUS, fluorescent bronchoscopy etc are introduced subsequently to make the institute international standard. Anaesthesia department has been providing one lung & two lung anesthesia, thoracic epidural technique, etc. for post operative management & to facilitate the specialized thoracic surgery concerned. I hope it will be very useful for all concerns.We are trying with our utmost sincerity to make significant contribution.

Important information and necessary issues you want to raise
Last year inspite of our many limitations we gained a lot of success with our maximum effort in health status. This can be improved further - By increasing the existing bed (600 to 1500) capacity. Also increasing the manpower at all level. By fulfilling the vacant post of doctors and other staffs. This institute mostly need separate Radiology, Pathology, Molecular Diagnostic, Bronchoscopy, Surgery, OT, Post-Operative ward, Post-Operative ICU, Rehabilitation Unit, Smoking Cessation Program, Respiratory Lab, Allergy Diagnostic and Treatment Facilities. By creating a separate MDR building for MDR TB patients (very urgently necessary as because it is the only institute where MDR TB is treated). Beside this education and chest related research program need a separate building. By increasing the existing facilities namely- No. of ICU beds. No. of operation theaters. Modernize and improvement of hospital services and also identification of diseases more accurately several heavy equipment will be needed for this institute which are as follows- Name. of equipments: Simulator for FOB, EBUS facilities, Interventional Video-bronchoscope, Slim bronchoscope, Medical Pleuroscope, Endo bronchial Laser yag, Argon plasma coagulator, ETO Steriliser, CT SCAN 256, PET-Scan, Tracheal and Bronchial Covered Stent 20+20, Y stent, Video Thoracoscope, Mediastinospe, Portable X Ray Machine, 1000ma Digital X-Ray Machine, C -ARM X-Ray Machine, Cath lab for pulmonary angiogram & bronchial artery angiogram & intervention. Sufficient manpower to run the pathology lab for 24 hours. Transportation facilities need for emergency doctors & nursing staff. Chest & pulmonary diseases oriented special nursing institute should be immediately established for improvement of specialized nursing services. In this way if we overcome all these obstacles, this institute will be a center of excellence in this sub-continent.
